What are dermal fillers made of?

Most fillers used today are made from hyaluronic acid (HA), a sugar molecule that occurs naturally in your skin and binds water to add volume and hydration. Injectable HA gel restores volume that's been lost to aging or adds subtle contour where it's wanted. Other fillers, like calcium hydroxylapatite, work by both adding volume and stimulating your own collagen production.

How It Works

HA fillers act like a scaffold, physically restoring lost volume and structure beneath the skin. Because HA also binds water, it adds a hydrating effect to the treated area. Some fillers go a step further: calcium hydroxylapatite (Radiesse) and poly-L-lactic acid (Sculptra) work partly by stimulating your own body to produce new collagen over time, so the improvement continues to build after the initial volumizing effect.

What to Expect

Treatment typically takes 15–30 minutes, often with topical or injected numbing for comfort. Results are visible immediately, though mild swelling can temporarily exaggerate the effect for the first few days. Bruising is possible but usually resolves within a week to ten days. Most patients return to normal activities the same day.

How Long Do Fillers Last?

Most HA fillers last 12 to 24 months depending on the treated area, product used, and individual metabolism — denser areas like the cheeks tend to last longer than more mobile areas like the lips. Interestingly, newer imaging research suggests HA may persist in the skin longer than once believed, even after visible effect has faded, which is part of why many patients need only a smaller "top-up" rather than a full re-treatment.

Are Dermal Fillers Safe? What Are the Risks?

Common side effects — bruising, swelling, tenderness, redness — are mild and short-lived. Serious complications, such as a vascular occlusion where filler compresses or enters a blood vessel, are rare but require immediate treatment. This is why choosing an experienced, medically trained injector who understands facial anatomy and can recognize and manage a complication quickly matters as much as the product itself.

Can Filler Be Reversed or Removed?

This is one of the biggest safety advantages of hyaluronic acid fillers: an enzyme called hyaluronidase can dissolve HA filler if you're unhappy with results or if a complication needs urgent correction. This reversibility is not available with all filler types, which is one reason HA remains the most commonly recommended option for first-time filler patients.

How Long Results Really Last

What researchers found: A systematic review of clinical durability studies, along with recent 3D volumetric and MRI-based research, found HA filler persistence often extends well beyond the traditional 3–12 month estimate, with meaningful volume detectable at 12 and even 24 months in some patients.

Why it matters: This is reshaping how providers counsel patients on maintenance schedules — many patients may need less frequent touch-ups than previously assumed.

Patient Satisfaction Over Time

What researchers found: A study of full-facial volume restoration with HA filler found volume restoration was well sustained over 18 months, with high patient satisfaction and good tolerability throughout the study period.

Why it matters: Long-term, patient-reported satisfaction data helps set realistic expectations for how results feel and look as they gradually fade, rather than only measuring changes on a clinician's scale.

Understanding and Managing Rare Complications
